What would get this ad rejected?

The four flags that kill most insurance ads
1 · Personal attributes. Meta's most common insurance rejection: your ad can't assert something about the reader. “Are you over 60 and worried about…” is a violation. “Most people over 60 don't know this exists” is fine — name the group, never the reader.
2 · Guarantee language. “Guaranteed acceptance” on a product that isn't guaranteed issue is a misrepresentation — a state problem, not just a Meta one.
3 · Rates in cold ads. The moment you quote a price you've stopped running a generic ad and started running a product illustration — different expectations, and it converts worse anyway.
4 · Implied government affiliation. “Government-approved benefit” — unless it's a real named program, that sentence doesn't exist.
This tool flags likely problems. It does not approve your advertising and it is not legal or compliance advice. Your state's Department of Insurance sets the advertising rules you're bound by as a licensed producer.
